liderado por <strong>Sule Canberk<\/strong>, investigadora da <strong>Unidade de Investiga\u00e7\u00e3o RISE-Health<\/strong> e docente da Faculdade de Medicina da Universidade do Porto (FMUP), demonstrou que determinadas altera\u00e7\u00f5es moleculares som\u00e1ticas do tumor da tiroide est\u00e3o associadas a diferentes progn\u00f3sticos desta patologia em crian\u00e7as, adolescentes e jovens adultos.<\/p><p>O estudo publicado na European Thyroid Journal, uma revista de refer\u00eancia a n\u00edvel europeu, demonstrou que <strong>o perfil molecular do tumor da tiroide tem valor progn\u00f3stico independente da idade, do sexo e da dura\u00e7\u00e3o do seguimento, contribuindo para prever a evolu\u00e7\u00e3o cl\u00ednica destes doentes.<\/strong><\/p><p>Esta descoberta, de acordo com a especialista, indica que a idade <strong>n\u00e3o deve ser tratada apenas como um descritor demogr\u00e1fico na avalia\u00e7\u00e3o de n\u00f3dulos tireoidianos em crian\u00e7as e jovens<\/strong>, real\u00e7ando que as recomenda\u00e7\u00f5es atuais de tratamento desta patologia n\u00e3o estabelecem, de forma expl\u00edcita, uma estratifica\u00e7\u00e3o da abordagem dentro da popula\u00e7\u00e3o pedi\u00e1trica.<\/p><p>\u201cAs fus\u00f5es [dos genes] NTRK1\/3 e RET, mais prevalentes nos doentes mais jovens, associaram-se a desfechos menos favor\u00e1veis, enquanto o BRAF V600E, mais frequente na adolesc\u00eancia, apresentou um perfil de risco interm\u00e9dio; as muta\u00e7\u00f5es RAS mostraram uma tend\u00eancia favor\u00e1vel e, na nossa s\u00e9rie, todos os casos com altera\u00e7\u00e3o DICER1 tiveram uma resposta excelente\u201d, revela Sule Canberk (RISE-Health\/FMUP).<\/p><h3>\u00a0<\/h3><h3>O futuro do diagn\u00f3stico e tratamento<\/h3><p>Segundo o trabalho cient\u00edfico, a caracteriza\u00e7\u00e3o molecular de tumores da tiroide em fases precoces da vida pode contribuir para uma estratifica\u00e7\u00e3o de risco mais precisa, abrindo caminho a decis\u00f5es cl\u00ednicas mais informadas, nomeadamente ao n\u00edvel do seguimento e da personaliza\u00e7\u00e3o terap\u00eautica.<\/p><p><strong>\u201cNeste estudo, demonstr\u00e1mos que o tipo de altera\u00e7\u00e3o molecular mant\u00e9m valor progn\u00f3stico independente mesmo ap\u00f3s ajuste para idade, sexo e dura\u00e7\u00e3o do seguimento\u201d, <\/strong>esclarece a patologista end\u00f3crina.<\/p><p>De acordo com a tamb\u00e9m docente da FMUP, \u201ca integra\u00e7\u00e3o sistem\u00e1tica da an\u00e1lise molecular poder\u00e1 melhorar a estratifica\u00e7\u00e3o do risco e apoiar decis\u00f5es cl\u00ednicas mais informadas. Tal permitir\u00e1 ir al\u00e9m da idade e do estadiamento cl\u00ednico e estimar melhor o risco com base no comportamento biol\u00f3gico do tumor\u201d.<\/p><p>\u201cCom base nestes resultados, estamos atualmente a desenvolver uma ferramenta algor\u00edtmica com o objetivo de refinar a abordagem pr\u00e9 e p\u00f3s-operat\u00f3ria dos doentes atrav\u00e9s do uso combinado de faixas et\u00e1rias de desenvolvimento, citologia, histopatologia, perfil molecular e dados de seguimento cl\u00ednico\u201d, conclui a investigadora.<\/p><p>Intitulado \u201cThe influence of age-independent somatic driver alterations on clinical outcomes in paediatric and young adult thyroid cancer\u201d, o estudo liderado por <strong>Sule Canberk<\/strong> (RISE-Health\/FMUP) teve <strong>Fernando Schmitt<\/strong> (RISE-Health@RISE\/FMUP) como coautor.
