sexualmente transmiss\u00edvel bacteriana mais comum a n\u00edvel mundial<\/strong>, com incid\u00eancia particularmente elevada entre adolescentes e jovens adultos, tendo-se registado, entre 2013 e 2023, um aumento geral de 13% no n\u00famero de infe\u00e7\u00f5es.<\/p><p>De forma a compreender melhor esta problem\u00e1tica e o seu impacto, um grupo de investigadores da Unidade de Investiga\u00e7\u00e3o <strong>RISE-Health<\/strong> e da Faculdade de Medicina da Universidade do Porto analisou o papel da microbiota cervicovaginal na sa\u00fade do trato reprodutor feminino.<\/p><p>A microbiota cervicovaginal \u201c\u00e9 um ecossistema din\u00e2mico de microrganismos que reveste a vagina e o colo do \u00fatero e que funciona como uma verdadeira \u201clinha da frente\u201d de defesa do trato reprodutor feminino\u201d, explica Nuno Vale, investigador principal do Grupo de Medicina Personalizada da RISE-Health@RISE\/FMUP, acrescentando que \u201cal\u00e9m da barreira f\u00edsico-qu\u00edmica, esta microbiota modula a resposta imunit\u00e1ria local, influencia a inflama\u00e7\u00e3o da mucosa e pode impactar processos como a ascens\u00e3o de microrganismos para o \u00fatero e trompas, a fertilidade, a implanta\u00e7\u00e3o embrion\u00e1ria e os desfechos da gravidez\u201d.<\/p><p>\u201cQuando esta microbiota se encontra \u201cdesorganizada\u201d (disbiose), por exemplo, com perda de <em>Lactobacillus<\/em> e aumento de anaer\u00f3bios como <em>Gardnerella<\/em>, <em>Prevotella<\/em> ou <em>Atopobium<\/em>, aumenta o risco de infe\u00e7\u00f5es sexualmente transmiss\u00edveis, incluindo <em>Chlamydia trachomatis<\/em>, e de complica\u00e7\u00f5es a montante, como doen\u00e7a inflamat\u00f3ria p\u00e9lvica, infertilidade tub\u00e1ria e gravidez ect\u00f3pica\u201d, esclarece o tamb\u00e9m Professor Associado da Faculdade de Medicina da Universidade do Porto (FMUP).\u00a0<\/p><p>Segundo o estudo publicado no <a href=\"https:\/\/www.mdpi.com\/1422-0067\/26\/21\/10635\"><em>International Journal of Molecular Sciences<\/em><\/a><em>, <\/em>\u201ca composi\u00e7\u00e3o da microbiota cervicovaginal varia ao longo da vida da mulher, influenciada por v\u00e1rios fatores fisiol\u00f3gicos, hormonais e ambientais\u201d, apontando que uma flora cervicovaginal dominada por <em>Lactobacillus<\/em>, tamb\u00e9m conhecida como \u201cc\u00e9lulas boas\u201d, particularmente enriquecida em <em>L. crispatus<\/em>, \u201capoia a integridade da barreira epitelial, limita a inflama\u00e7\u00e3o e favorece a elimina\u00e7\u00e3o espont\u00e2nea de pat\u00f3genos\u201d.<\/p><p>Na vis\u00e3o dos especialistas, \u201cas interven\u00e7\u00f5es centradas na microbiota s\u00e3o promissoras porque podem aumentar a imunidade natural, melhorar a efic\u00e1cia das vacinas, reduzir a morbidade associada \u00e0 clam\u00eddia genital e at\u00e9 mesmo reduzir a incid\u00eancia de outras infe\u00e7\u00f5es sexualmente transmiss\u00edveis\u201d.<\/p><p>\u201cImplementar estrat\u00e9gias de rastreio mais estruturadas, em particular em adolescentes e jovens adultos sexualmente ativos\u201d, \u201cfacilitar o acesso ao diagn\u00f3stico no primeiro n\u00edvel de cuidados\u201d, \u201cgarantir tratamento adequado e atempado, de acordo com as normas e <em>guidelines<\/em> europeias e nacionais\u201d, \u201crefor\u00e7ar a notifica\u00e7\u00e3o e tratamento de parceiros sexuais\u201d e o desenvolvimento de \u201cprogramas de educa\u00e7\u00e3o sexual e literacia em sa\u00fade, que integrem informa\u00e7\u00e3o sobre IST, import\u00e2ncia do rastreio, uso de preservativo e impacto da clam\u00eddia na fertilidade feminina e masculina\u201d devem estar entre as medidas a adotar para combater o aumento da infe\u00e7\u00e3o por clam\u00eddia genital.<\/p><p>Al\u00e9m do investigador Nuno Vale, o artigo \u201c<em>Disrupted Cervicovaginal Microbiota: Its Role in Chlamydia trachomatis
